Urgent & Emergency Adult Cardiac Surgery Services
Our policy dictates the prompt acceptance of urgent & emergency patients who may need cardiac surgery. Patients with demonstrated surgical problems are accepted directly under cardiac surgeons in a cardiac surgical ward. We are keen to undertake extra investigations in our centre, i.e. carotid Doppler, ECHO (transthoracic or transoesophageal), CT scan, and MRI test for viability and hibernation.
Elective Adult Cardiac Surgery
Our unit provides high quality services in the entire spectrum of Adult Cardiac Surgery. This includes:

Coronary Artery Bypass Grafting: The short and long-term results of coronary revascularization in our unit are outstanding. Complete Arterial Revascularization and bilateral Internal Mammary Arteries are used when appropriate.

Aortic Valve Surgery: Our unit has shown a special interest in Aortic Valve surgery for a number of years. We have used the larger variety of stentless valves and this has been acknowledged world-wide.

TAVI: Surgical valve replacement is the standard therapy for fit patients. However, in unfit patients surgery may be associated with a very high risk. The advent of transcatheter valve replacement techniques by means of percutaneous or transapical approaches has been recently introduced into mainstream clinical practice and is likely to radically change the treatment of aortic valve disease. By June 2011 120 TAVI cases have been performed at Glenfield Hospital with the one of the lowest reported mortality rates.

Sutureless Aortic Valves: More recently, we have become one of the two approved centres for the Sutureless Aortic Valve trial. This trial began in July 2010 and targets patients who are rejected by the TAVI team due to technical issues. We successfully implanted the first Percival Sutureless Aortic Valve in July 2010.

Mitral Valve Surgery: Our unit has been one of the most highly specialized centres for Mitral Valve Repair. It holds a national training course for standard and complex Mitral Valve Repair.

Surgery for Aorta: Our unit has a long tradition with the treatment of diseases of the ascending aorta. Since 2004 we have shown a specialized interest for dissection of the aorta. We have actively participated in the front line of the National campaign for commissioning this type of surgery to specialized centres. Our results in acute dissection of the aorta type A are among the best in the country.

Adult Congenital Cardiac Surgery: Our unit has an established practice for Adult Congenital Diseases.
Partnership promotion
Our surgeons participate in cardiology MDT’s at surrounding District General Hospitals and also hold clinics at these Hospitals at least once a month when space is available. We have one of the most accurate surgical audits, monitoring early and late mortality.
Theatres & Intensive Care Unit
We have 3 operating theatres serving Adult Cardiac Surgery. We have 18 beds in the intensive care unit serving Adult Cardiothoracic Surgery and ECMO.
The intensive care unit has 9 consultant intensivists, qualified in ICM with a cardiac or thoracic interest. The unit has attained advanced ICS training recognition. Consultant cover is provided 24/7 on a dedicated rota with continuity of care. The unit has its own U/S scanner providing point of care treatment at the patient bedside which then allows other modalities such as ECMO support/ nitric vent /CVVH/ IABP and transthoracic and transoesophageal ECHO to be managed effectively.
Future plans
The major targets of our unit include:
1. Increase annual activity from 1,200 to 1,400 cases per annum
- Appointment of two new consultants
- Expanding the theatre capacity from 26 sessions to 30 sessions per week
2. Expanding the intensive care unit
Volumes
From 2007 to the present we perform about 1,200 cases per year. Of these cases only 47% were first time CABG which shows that our unit accumulates more complex cases including aortic and mitral valve surgery, as well as complex surgery of the aorta and adult congenital cases.
Results
The Society of Cardiothoracic Surgeons in England has recognised our unit as the 8th largest unit in UK among 38 other units performing adult cardiac surgery. Our overall operative observed mortality is amongst the best in the UK.
Location
The Cardiothoracic unit is based at the Glenfield Hospital. It serves a population of 2 million and is a secondary and tertiary referral centre for adult heart surgery.
