Patient Passport

Patient Passport

If you are coming to see us, we want to learn as much about you as possible. Fill in this patient passport and bring it with you whenever you visit Leicester Children’s Hospital so we can learn important things about you.


The passport will be especially helpful for children and young people who have additional needs, but we hope it will benefit anybody that uses it.


You can do this by clicking on the link below, printing out the passport and filling it in. Then just bring it along with you next time, and every time, you come in. If you don’t have access to a printer then ask for an All About Me patient passport at the desk when you arrive for your hospital appointment or admission.

It doesn't take long to fill in, it's only two pages. We’ve also made a guidance sheet to help you understand the questions and fill in the passport.


Printing instructions for booklet style passport

 

We recommend printing in colour to benefit from the colour-coded system inside the passport. If you don’t have access to a colour printer, ask one of the staff members for a patient passport when you next visit Leicester Children’s Hospital 


  1. Download the patient passport at the bottom of the page
  2. Click the print icon in the top-right corner
  3. Click the button at the bottom of the menu saying ‘Print using system dialogue’
  4. Look at the Select Printer section and make sure a colour printer is selected
  5. Then click on Preferences 
  6. In the drop-down menu called ‘2-sided printing’ select ‘2-side Print, Flip on Short Edge’
  7. Click OK
  8. Click Print
