Verification of Identity Evidence of Entitlement to Work in the UK in line with the Immigration, Asylum and Nationality Act 2006, if you have limited leave to remain in the UK the Trust has a legal requirement to check the continued validity of such at regular intervals | A valid Passport and two further proofs of the following identification: - Driving Licence
- Bank Statement or Utility Bill dated within the last 3 months
- Mortgage or Council Tax Statement dated within the last 12 months
- P60 or Marriage Certificate

Evidence of Professional Registration and Qualifications | An original valid GMC Certificate and relevant qualification certificates |
A Disclosure and Barring Service Certificate (formally known as CRB) | A valid DBS certificate (must be within the last 3 years) or a new DBS will be required to be undertaken. The Trust will commence the applications and you will be required to complete the remainder of the form on-line. The cost of the DBS will be taken from your pay over a 3 month period. Please note you can subscribe to the DBS update service for an annual fee of £13.00. You will need to register online when completing your DBS or within 19 days of receipt of certificate (www.gov.uk/dbs-update-service) |

You are also required to complete your online e-induction covering relevant induction issues, most of your mandatory training and formal assessment of your learning before your first day with the Trust. Completion of this training will entitle you to a day off in lieu if completed before your start date. Further information can be found in the 'Statutory and Mandatory Training' section.
