Our Hospital Specialist Palliative Care Team consists of a hospital-based team based at the Royal Infirmary, General and Glenfield Hospitals. The team provide a service to adults with palliative care needs within our hospitals and also in the outpatient clinic setting.
The medical and nursing teams on the ward remain responsible for the person (patient’s) care and should be aware of all referrals to the Hospital Palliative Care Team.
Referrals to the Palliative Care Team should usually be made by the ward teams, who are able to do this via the electronic referral system (the ICE clinical system).
If you are a patient or family member and wish to speak to the palliative care team, please ask the ward to make a referral. If you still feel that you need additional support, please ask to be put through to the Palliative Care Team at the hospital site where the person is an inpatient. Please see contact telephone numbers within the 'Our Teams' section.
