Bereavement Services

Image of Blue Butterfly

Our Bereavement Service helps relatives/carers when a patient has died in one of our hospitals. They guide them through the practical aspects following the death of a loved one.

Our staff co-ordinate the paperwork that needs to be completed after death, such as the "Medical Certificate of Cause of Death" and any necessary cremation forms.  They will liaise with medical and nursing staff, funeral directors, the coroner’s office, the registrar's office and any other outside agencies.

They will give sensitive and compassionate care to all bereaved families.  They are aware of religious and cultural issues around the time of death and will do their best to accommodate the needs of all families.

They also deal with lost property found in the hospitals.

Information about bereavement

Our Bereavement Services Team have produced the information leaflet at the bottom of this page. We hope it will assist you during the early days of your bereavement.

The booklet provides help and advice with the practical issues associated with the loss of a relative or friend, particularly if the death has occurred in hospital.

Please do not hesitate to ask if you are unclear about anything. We will always be pleased to help.

Bereavement Support Service

The Trust also provides a Bereavement Support Service which is available from 9am - 5pm Monday to Friday (excluding Bank Holiday’s).

If you have suffered a bereavement within one of our three hospitals we are here to:

  • Provide further information or support.
  • Offer the opportunity to talk through what matters to you regarding your bereavement.
  • Arrange for you to meet with a member of the Medical / Nursing team who cared for your loved one, if you have questions you would like to raise and discuss.

If we are unable to help, we will signpost you to someone who can.  For example, although we do not provide a counselling service, we can assist you in finding someone who can help.

Family members and carers may contact us at any time. Family members are also offered the opportunity to receive a follow up telephone call or letter six to eight weeks after their bereavement.

If further information or contact is required in another language or format, this can be arranged.

Telephone: 0116 258 4380 or 07950 868337
