Chronic Kidney Disease

The fourth in a series of podcasts includes discussion on, improving patient care and awareness of Kidney Disease progression. Together IMPAKT  is an innovative and integrated programme of work taking place in primary care to help identify and prevent patients from developing progressive chronic kidney disease (CKD). The programme is a collaboration with the CLAHRC for Greater Manchester and the CLAHRC for Leicestershire, Northampton & Rutland. Early detection and effective management of CKD can lead to much improved outcomes for patients.
